Home & Community Care
Home and community care is a service for eligible residents in the Gungahlin/Hall area who are senior, frail, aged, have a disability and for their carers. Eligibility is determined by HACC Case Managers.
The range of services available include:
Community Transport: 3 different services available for eligible people.
Transport Service 1: For attending medical, paramedical (including injury) and social activities. Bookings 3 days in advance, Monday to Friday between 9:30am and 3:00pm, unless it is an emergency. HACC eligibility criteria* applies to all users of this service.
Transport Service 2: The community bus service is for people who are socially isolated because of a lack of transport options.
Transport Service 3: Supported Shopping. Fortnightly to Gungahlin Marketplace, alternate Mondays and Tuesdays.
Some of the people who may be eligible to use one or both of these services may include:
- Seniors with mobility or confidence problems*
- People living in nursing homes / retirement facilities
- People with a permanent or temporary disability*
- People with health problems who cannot use regular ACTION bus services
- People who are eligible for HACC transport but cannot access them for various reasons e.g. cannot self transfer
- Carers accompanying a person described above*
- Parents with young children who are socially isolated and lack transport
- People from culturally and linguistically diverse backgrounds who lack support networks and have limited transport options
To become a registered user of the transport services you need to be assessed by Home and Community Care. Contact a Case Manager on 6242 6077 for a time for an assessment and for details on the service costs.
