DISABILITY COORDINATOR:
This position will be responsible for the coordination and delivery of front services and the rostering of staff for the GRCS Disability Services. The position is part time - 50 hours per fortnight. HOME & COMMUNITY CARE (HACC) CASE MANAGER:
This position will be responsible for the day to day service coordination for clients, including assessment, referrals and care planning. The position is part time - 50 hours per fortnight.
